Industry spent nearly $3.5 billion on such pay- ments in the five-month pe- riod from August through December of last year, ac- cording to the Centers for Medicare and Medicaid Services, which released data on 4.4 million pay- ments. The massive trove of in- formation named compa- nies and many of the re- cipients, although in about 40 percent of the records, identifying details were re- moved due to unresolved questions. Also listed were types of payments, with de- tails down to travel desti- nations. Some 546,000 cli- nicians and 1,360 teaching hospitals received pay- ments. Some also had own- ership stakes in companies. It's all part of a new ini- tiative called Open Pay- ments, mandated by Presi- dent Barack Obama's health care law. As conceived, Open Payments was in- tended to allow patients to look up their own doctors online, but that functional- ity isn't fully ready. In fu- ture years, the information will cover a full 12 months and will be easier to search, officials said. Still, the preliminary data are expected to be useful for professional re- searchers. About 300,000 records are still being veri- fied, and were not included, so the total value of pay- ments is expected to be higher. Consumer groups ap- plauded the move as a much-needed step toward greater transparency. But doctors and industry said the government rushed to release the data, and raised questions about accuracy and lack of context. Obama's administra- tion has seen a policy shift to open the books of the medical profession. Unlike his health care law, Open Payments is supported by members of both political parties. A few months ago Medicare released its huge claims database, showing program payments to more than 825,000 providers for 2012. Drug companies tradi- tionally have prized their relationships with doctors, the gatekeepers between patients and prescription medications. But consumer groups see a built-in con- flict of interest that could influence prescribing deci- sions. Some doctors' offices have started curbing phar- maceutical marketing, which can include any- thing from free pens and pizzas to paid speaking en- gagements and golf outings at fancy resorts. But many doctors also receive signifi- cant payments to help drug companies conduct clinical research. The American Medical Association says it is "ex- tremely concerned" about release of the payments file, adding that the data may contain inaccuracies and lacks context to help the average person evaluate the information. The fed- eral Centers for Medicare and Medicaid Services pro- vided individual doctors an opportunity to inspect their data prior to release, but the AMA says the window was short and the process cum- bersome. Consumer groups call the disclosure program the "Sunshine Act," and say it's overdue. "Research has shown over and over that these fi- nancial relationships influ- ence doctors, even a meal," said John Santa, medical director for health projects with Consumers Union. "Studies also show that doc- tors believe it does not af- fect them, but strongly be- lieve it affects other doc- tors." AMA President Robert Wah said in a statement that some financial deal- ings between doctors and the drug industry help pa- tients, particularly when it comes to research. There are "appropriate relation- ships in this realm that can help drive innovation in pa- tient care," Wah said. President Barack Obama has pressed for the agree- ment, but his comments about the situations in the two countries have brought criticism from some quar- ters. While he has touted the Afghan accord as cru- cial to protecting progress in the fight against al-Qaida, he's also insisted that had he reached a similar pact with Iraq, it would have done lit- tle to stop the rise of the Is- lamic State militants wreak- ing havoc there and in neigh- boring Syria. "The only difference would be we'd have a bunch of troops on the ground that would be vulnerable," Obama said in August, shortly after authorizing airstrikes in Iraq. "And how- ever many troops we had, we would have to now be rein- forcing, I'd have to be pro- tecting them, and we'd have a much bigger job." The president and his ad- visers have repeatedly said they had no choice but to withdraw from Iraq. Un- der an agreement signed by former President George W. Bush, U.S. troops had to leave by the end of 2011 unless an extension was signed. But White House critics, as well as some former ad- ministration officials, have suggested that Obama put his desire to end the Iraq war ahead of concerns about the security vacuum the U.S. might leave behind.
