DESCRIPTION OF PROFESSIONAL SERVICES DESIRED - The services desired by the City pursuant to this RFP and provisions related to it are described in the follow- ing exhibits, which are incorpo- rated herein by this reference. See website at www.cityofredbl uff.org, or email sryan@cityofre dbluff.org for full RFP, including Exhibits. • EXHIBIT "A" - GENERAL DE- SCRIPTION OF PROJECT • EXHIBIT "B" - SCOPE OF CON- SULTANT SERVICES • EXHIBIT "C" - COMPENSATION • EXHIBIT "D" - SPECIAL PROVI- SIONS • EXHIBIT "E" - SELECTION CRI- TERIA MATRIX PROPOSAL AWARD/REJECTION - A proposal award, if a proposal is awarded, will be made to the most responsible professional services provider whose propos- al complies with the City's re- quirements, as set forth herein within seventy-five (75) days of the above proposal opening date. The City may interview se- lected proposers. The City re- serves the right, in its sole dis- cretion, to reject any proposal which fails to meet the proposal requirements in any respect, to reject all proposal(s) for any reasons whatsoever and to waive minor irregularities in any proposal. PROPOSAL SUBMITTAL REQUIRE- MENTS - All proposals shall be in compliance with the following requirements. Failure to comply with the requirements shall be grounds for the rejection of a proposal. 1. A written proposal shall be submitted, signed by an author- ized individual of the professio- nal services provider. 2. The cost for required serv- ices shall accompany all propos- als. Proposals shall include a supported breakdown of costs and the consultant's hourly rates to be used for determining compensation. 3. Receipt of and agreement with all exhibits incorporated in the RFP shall be acknowledged. 4. The proposal must be sub- mitted in a sealed envelope plainly marked on its outside with the project title: Actuarial Valuation for Retiree Health In- surance for GASB 45. BUSINESS LICENSE - The profes- sional services provider, and any subcontractor(s), shall ob- tain a valid, current City of Red Bluff Business License on or be- fore their commencement of work. PROFESSIONAL LICENSING - The professional services provider, and any subcontractor(s), shall possess any necessary profes- sional certification(s) and/or license (s) relative to the work to b f d i d b ( ) b e performed required by an ap- propriate licensing authority of the State of California and must provide evidence of such to the City with their proposal or prior to commencement of the work in such form as the City shall re- quire. INDEMNIFICATION - The provider to who a contract is awarded will be required to indemnify and hold the City harmless from any and all liabilities arising from the provider's performance of the work. Indemnification provisions to this effect will be incorporated in the Professional Services Agreement which will be executed between the pro- vider selected for the project and the City. INSURANCE - The provider to whom a contract is awarded will be required to furnish to the City evidence of insurance coverage(s) including, but not limited to, general liability, pro- fessional liability, and workers compensation insurance, as ap- propriate. PAYMENT RETENTION - A ten percent (10%) retention will be withheld from each invoice until submittal of all project docu- ments are approved by both the City and the participating state agencies. DISCLOSURE - CONFLICTS OF IN- TEREST - The professional serv- ices provider selected may be required to disclose potential conflicts of interest pursuant to the provisions of the City's Con- flict of Interest Code.
