FEATURES Dear Annie: My mother is in her late 90s. She insists on covering her kitchen counters with newspaper, including the areas right next to the electric cooktop and the microwave. My mother does not see this as a fire risk. She thinks it is keeping a clean house. The last time I cleaned her kitchen, I was disgusted by the small insects and bits of food that were trapped between the newspaper sec- tions. Her housekeeper simply goes along with her wishes and leaves the newspapers where they are. But, Annie, if this paper catches fire, Mom would never make it out of the house alive. I am not sure why she insists on putting her food on top of newsprint. She says newspapers are sterile. But the paper is handled by many people and sits in delivery trucks that are hardly spotless. Those who deliver it aren't wearing rubber gloves. All of this escapes my mother's attention, and she just piles the papers on her counters, which are, by the way, in great shape because she never actually uses them. Mom reads your column every day and follows your advice. Any paper that rests near an electric cooktop (or any cooktop) is a risk. Mom — if you're reading this, please wash your counter- tops instead of covering them with old newspapers. Your daughter wants you to be healthy and safe. So do we. Dear Annie: I have worked hard all of my life and now find that I am financially very well off and can afford to be generous with my money. We think they will fall all over themselves thank- ing you, as well they should. Since I believe charity begins at home, I sent two $1,000 checks to two nieces who are going to college. It has been more than a month, and I have not heard anything from them. I even emailed the mother of one, thinking the check had gotten lost because it wasn't cashed. It was then deposited, but still no thank- you note. Angioplasty, stent ensure better blood flow DEAR DOC- DEAR READER: In yesterday's column I dis- cussed coronary angiogra- phy. That diagnostic test allows doctors to detect blockages from plaques of atherosclerosis in arteries that supply blood to your heart. As in your case, if a doctor finds a blockage dur- ing an angiography, he or she may perform an angio- plasty and place a stent to open up the blockage. (These procedures are often performed immediately after a heart attack.) TOR K: I recently had an angiogram, and during the pro- cedure the doctor found a blockage. He performed an angioplasty and placed a stent. He said my angina should go away, and it has, but I real- ly don't understand what he did. Can you explain it? day, to do the angiogram your doctor pushed a soft tube (a catheter) from an artery in your leg or arm up to your heart. Squirting dye through the catheter into the arteries of the heart revealed at least one blockage. Your doctor decided there was a good chance the blockage could be opened by an angioplasty, using the catheter. (Not all blockages can be reached or safely opened by a catheter.) Here's how an angioplas- As we described yester- ty works: Inside the catheter used to perform the angiogram is a thinner catheter that has an inflat- able balloon near its tip. And inside that catheter is an even thinner wire with a soft tip. The doctor watches an Dr. K by Anthony L. Komaroff, M.D. Once the wire has crossed the blockage, the doc- tor slides the catheter with the little uninflated balloon at its tip down the wire until the balloon is right where the blockage is. Then the physician inflates the bal- loon. This cracks and com- presses the plaque, squash- ing it against the wall of the artery and making the inside of the artery wider. As a result, blood flows through the area more easily. Doctors' early experi- ence with angioplasty found that the widening of an artery was often temporary. Once the catheter reaches the heart, the doctor guides the wire down the artery until the wire tip is beyond the block- age. devices. The tubes start with a very narrow center that just fits over the uninflated balloon. When the balloon is inflated at the blockage, the stent also expands. It stays expanded, keeping the plaque squashed against the wall of the artery, allowing good blood flow through the artery. The doctor with- draws the catheter and the deflated balloon, but leaves the expanded stent in place. Most stents are coated with a drug that helps to prevent renarrowing of the artery.
