The Senior Nutrition Program serves meals Monday through Friday at noon at the Red Bluff Community-Senior Center and 11:30 a.m. at the Corning Senior Center. The suggested donation for seniors 60 and older is $2.75. For those guests under the age of 60, the guest charge is $7. A donation is not a requirement for you to receive a meal if you qualify for the program. Reservations must be made a minimum of one day in advance by calling 527-2414. The ClarkParkonFigLane: Two males in the area on Thursday reportedly broke into vehicles and stole items. POLICE LOGS Atlas, a 3-year-old male mastiff, was experiencing a decreased energy level and his respiratory rate was increasing more than normal, even on the eas- iest of walks. His owners were concerned enough to bring him to their vet- erinarian, who diagnosed him to have a build-up of a milky white fluid called chyle, which leaks from the thoracic duct, in his chest, a condition known as chylothorax. As this can be deadly if not treated properly, Atlas' veterinarian referred him to the specialists at the UC Davis Veterinary Medical Teaching Hospital. Once at UC Davis, At- las was immediately seen by the Emergency Service who removed almost 2,000 milliliters of fluid from his chest. The removal of fluid helped Atlas temporarily feel better, but the cause of the chylothorax needed to be determined. A number of things can cause chylothorax, including: tumors in the chest cavity, heart disease, trauma to the chest caus- ing damage to the tho- racic ducts and fluid leak- age, heartworm disease, and idiopathic chylotho- rax — in which no known underlying cause is iden- tified. Veterinarians discussed several options with Atlas' owners, including a plan for medical therapy that involved medications and an alteration of Atlas' diet, as well as a surgical option that involved thoracic duct ligation. As medical ther- apy is not generally very successful in resolving the chylothorax, Atlas' owners chose to have him undergo thoracic duct ligation. To prepare for the sur- gery, Atlas underwent general anesthesia for a CT lymphangiogram performed by the Diag- nostic Imaging Service, which outlines the num- ber and position of the thoracic ducts that carry chyle in the chest and helps the surgeons plan which side of the chest should be approached. Drs. Philipp Mayhew and Michele Steffey, of the Soft Tissue Surgery Service, were then able to proceed with the tho- racoscopic thoracic duct ligation. This minimally invasive approach to chy- lothorax is used in hu- man medicine and at a few veterinary schools, but what sets UC Davis' approach apart is the use of intraoperative near-in- frared imaging offered by Steffey to make the lym- phatic vessels more visi- ble to the human eye dur- ing surgery. The thoracic ducts that carry the chyle are very small, and often diffi- cult to see at surgery un- der normal illumination, as they are buried in fat and other tissue. How- ever near-infrared imag- ing uses different wave- lengths of light that is not visible to the human eye without special imaging systems. However, when sur- geons switch from a nor- mal view to the near-in- frared view through the scope, the near-infrared dye within the ducts al- lows them to see the posi- tion of the thoracic ducts very clearly, enabling the dissection and minimiz- ing any chance of missing the ducts. This approach enabled surgeons to eas- ily locate where Atlas' thoracic duct was leak- ing chyle into his chest cavity, and stop the leak- age. The near-infrared approach, pioneered at UC Davis by Steffey for this disease, may soon become the optimal pro- tocol for these types of cases. Other schools of veterinary medicine have taken notice, with a few recently purchasing the equipment to start us- ing the near-infrared method. After successful one- week and one-month re- check examinations, At- las was given a clean bill of health, and continues to do well nearly a year after the surgery.
