DEAR DR. GOTT: In Novem- ber 2009, I pulled a rib muscle in my back while working out in the gym. It later became abscessed and subsequently burst, discharging a cloudy liq- uid. The following month, I saw a surgeon who thought it could be infected. He cut into the wound to remove the fluid and bad tissue. At home, we had to pack the wound and cover it two times a day. This created tension with my wife, as she had to do it because I could not reach the area. In June 2010, the sur- geon decided the wound was not healing. He sug- gested that he reopen it, remove additional tissue and sew it up, turning it into a primary wound. The initial wound was 5.5 centimeters deep by 5 centimeters long and 4 Dr. Peter Gott centimeters wide. Now it is going to be made even larger. We met with another sur- geon who sug- gested he would make the wound even bigger and “dishpan” it so it would be eas- ier to pack. I’m at my wits’ end. I have had two open-heart surgeries that healed in weeks. Six months later, I’m starting all over. I still have no infection, but no one can seem to get the wound to heal. Do you have any advice? DEAR READER: There are a number of reasons why wounds do not heal, but it often gets down to a lack of blood supply. Blood transports oxygen, platelets and plasma to heal wounds. That’s why people with diabetes (diabetic ulcers), infections (from bacteria or foreign bodies), smok- ers (vascular disease) and a host of other conditions have difficulties healing wounds. Get a referral to a top- notch surgeon at a local, well-respected hospital. Explain the length of time you have had the open wound and the cir- cumstances leading up to it. If there is an underly- ing cause hampering your recovery, be sure to address and correct it. Then ask about treatment with IV antibiotics; vacu- um-assisted closure; stem-cell therapy, which can provide the injured tissue with cells that can develop into healthy tis- sue; and skin grafting. Millions of dollars are spent every year because of nonhealing wounds. Add that to the trauma you and many other peo- ple suffer on a daily basis, and it’s hard to put a price on. You need the help of a wound-care spe- cialist, who can deter- mine the reason your wound isn’t healing and take the necessary steps to ensure it does. GOTT: My husband uses self- catheters four times a day, voiding into a urinal he holds between his legs. He uses a baby wipe before and after on his hands and rinses the urinal. He believes this is enough. I wish he would wash his hands under running water after putting his urinal away. Is there any way to convince him to do this? DEAR READER: Try a compromise. Ask him to wash his hands under running water before coming to the dinner table, using the tele- phone, touching door- knobs, and after petting the dog. The company happens to include Sally, the best apple pie maker in the western United States. And she brings us...an apple pie. Sometimes we just eat more later in the day... Does that matter? Does the timing of our meals and snacks affect our health in any way? Experts have been investigating this for a long time. I volunteered for one such study back in my college days. As part of a weight loss research study, a group of us college students were instructed to eat 1000 calories a day. Half of us (my group) ate all our daily calories in the morn- ing — nothing to eat after 1 pm daily. The other groups ate all their calories in the afternoon and evening — after 1 pm. The results? Even though both groups ate the same num- ber of calories, my group — the early in the day eaters — lost more weight. This study also taught me that breakfast tastes really good when you go to bed a bit hungry. Recent studies, too, have reported greater weight loss when meals are eaten earlier in the day. Interesting, howev- er, these same studies also report that lean body tissue (mus- cle) was better preserved by eating later in the day. What about the frequency of eating meals? Just for con- versation, let's say I'm going to eat a large piece of apple pie. Does it matter to my health if I eat it all at once or divide it into 3 smaller meals during the day? One study found no difference in weight between people who ate 3 meals a day versus one (if calories were the same). However, the volunteers who ate just one meal a day reported feeling more hungry during the day. Cholesterol and glucose levels were also somewhat higher in the one- meal-a-day group. So should I just nibble willy-nilly on apple pie through- out the day? Probably not. Studies show that people who eat at irregular times (6 or more times a day at various intervals) tend to burn fewer calories after these meals than folks who eat at more regular times. And...I know from experience that when I eat too often, I eat too much. In general, studies seem to support that eating at regular intervals helps curb hunger and may even help the body burn fat for energy more efficiently. Regular meals (and snacks when needed) help keep the body perking (burning calories) more profoundly than irregular meals. And eating earlier in the day seems to have advantages over late night gorging.
