Dear Annie: ''Doing It Offer her the DVDs and say that Mailbox Myself'' asked for advice about his mother, who has dementia. She wanted to stay in her home. I am a retired Adult Protective Services social worker whose job was to investigate adult abuse, neglect and exploitation. I cannot tell you how many times I investigated a report in the home and knew as soon as I saw the caregiver that the person had a history of violence. I have told my children that I want to remain in my own home only if I can recognize it as my own and the information I give them is reliable. (Dementia patients often report thievery or abuse when it does not exist.) Otherwise, I want to be in a nursing home. There is a much greater chance that abuse or neglect will be witnessed in a nursing facility. When the patient's family hires a caregiver, it is important that they go through a licensed reliable service that screens their employees. They should never look through the classified ads for an individual to care for a loved one. — Been There Annie's Mailbox is written by Kathy Mitchell and Marcy Sugar, longtime editors of the Ann Landers column. How to get a copy of a prior year tax return There are many reasons why you should keep a copy of your federal tax return. For example, you may need it to answer an IRS inquiry. You may also need it to apply for a student loan or a home mortgage. If you can't find your tax return, the IRS can provide a copy or give you a transcript of the tax information you need. Here's how to get your federal tax return information from the IRS: 1. Transcripts are free and you can get them for the current year and the past three years. In most cases, a transcript includes all the information you need. 2. A tax return tran- script shows most line items from the tax return you originally filed. It also includes items from any accompanying forms and schedules you filed. It does not reflect any changes made after you filed your original return. 3. A tax account transcript shows any changes either you or the IRS made to your tax return after you filed it. This transcript includes your marital status, the type of return you filed, your adjusted gross income and taxable income. 4. You can get transcripts on the web, by phone or by mail. To request transcripts online, go to IRS.gov and use the Order a Transcript tool. To order State sends Head of Household audit letters The Franchise Tax Board (FTB) announced mailing more than 100,000 audit letters to taxpayers to verify their "Head of Household" (HOH) filing status on their 2012 state tax return. Each year FTB reviews tax returns of taxpayers who claim the HOH filing status because the qualifications are commonly misunderstood. You can generally claim the Head of Household filing status on your tax return if you are unmarried, have cared for a qualified person for more than half the year, and paid more than half the cost of maintaining your home. Taxpayers who can claim this filing status benefit from a higher standard deduction and lower tax rates as compared to the single filing status. More than two million California taxpayers use this filing status each year. Taxpayers who do not qualify will have their tax reassessed at either the single or married-filing-separate filing status. FTB assessed $26 million in additional tax to the nearly 38,000 taxpayers who used this status last year and did not meet its requirements. FTB encourages taxpayers who receive a HOH letter to respond promptly by completing the enclosed questionnaire. Failure to respond could result in a penalty. Questionnaires can be submitted by any of these methods: • Respond electronically at ftb.ca.gov. Use HOH Audit Letter Web Responsepage. • Respond by fax at 866.223.8195. • Respond by mail using the pre-addressed envelope provided with the audit letter. FTB provides the following tools on its website to assist taxpayers: • Head of Household "self-test." • Answers to frequently asked questions. • Publication 1540, "CA Head of Household Filing Status," in English and Spanish. by phone, call 800-9089946 and follow the prompts. 5. To request a 1040, 1040A or 1040EZ tax return transcript by mail or fax, complete Form 4506T-EZ, Short Form Request for Individual Tax Return Transcript. Businesses and individuals who need a tax account transcript should use Form 4506T, Request for Transcript of Tax Return. 6. If you order online or by phone, you should receive your tax return transcript within five to 10 calendar days. You should allow 30 calendar days for delivery of a tax account transcript if you order by mail. 7. If you need an actual copy of a filed and processed tax return, it will cost $57 for each tax year. Complete Form 4506, Request for Copy of Tax Return, and mail it to the IRS address listed on the form for your area. Copies are generally available for the current year and past six years. Please allow 60 days for delivery. 8. If you live in a Presidentially declared disaster area, the IRS may waive the fee to obtain copies of your tax returns. Visit IRS.gov and select the 'Disaster Relief' link in the lower left corner of the page for more about IRS disaster assistance. 9. Forms 4506, 4506T and 4506T-EZ are available at IRS.gov or by calling 800-TAXFORM (800-829-3676). deep vein thrombosis have been born DEAR DOCwith a tendency TOR K: Last year for your blood to I developed deep clot more easily vein thrombosis than is the case for (DVT) — blood most people.
