(479) 751-7417 www.communityclinicnwa.org Cox Foundation - Serving Stone and Taney Counties in Missouri: "Health and Hope" provides access to breast care and breast cancer treatment through financial assistance for women in need of screening mammograms, diagnostic testing, and transportation to appointments. A nurse navigator is also provided to help those diagnosed with breast cancer navigate their journey. (479) 738-2612 www.madisoncountyhealth.com Mercy Fort Smith - Serving Crawford and Sebastian Counties: The "Save A Life" program provides navigation, screening and diagnostic services, educational outreach programs and works with diverse groups in the community to coordinate access to breast health services in the River Valley. (479) 314-5264 www.mercy.net/service/mammography North Arkansas Partnership for Health Education - Serving Boone, Carroll, and Newton Counties: "Living Healthy: Screenings for Life" program is designed to reach residents with a comprehensive education and awareness campaign to increase participation in breast health education classes, breast self-awareness training, and access to treatment. This program also provides screening and diagnostic services as well as patient advocacy and treatment assistance. The program guides patients through diagnosis, treatment, and into survivorship while linking them to financial and support recources as needed. The program also provides screenings, diagnostic services, and treatment support.
