Utility Assistance Pre-Enrollment Program for Seniors Documentation needed for Utility Assistance for Electricity: #1 Applicant's proof of U.S. Citizenship or lawful permanent residency. For example: birth certifi cate, certifi cate of naturalization, U.S. pass- port, Social Security Award Letter, Permanent Resident Card (Applicant MUST be Resident for more than 5 years to be eligible). #2 Applicant's Arizona State Driver's License / AZ DMV ID or Proof of AZ Residency (for example, Lease Agreement) and Picture ID. #3 Social Security cards of adults applying. #4 Documentation of gross income from ALL household members received in the 30 days prior to and including appointment date. For example: Social Security Award Letter, pension, etc. #5 Most recent electric bill. All Account holders must sign a Utility Re- lease Authorization at the time of the appointment. #6 If you are paying electric utilities to your Landlord: You must request from WACOG a Notice of Late Utilities form and a W-9 Form, which your Landlord must fi ll out.
