Tuesday, May 8, 2012 – Daily News REQUEST FOR QUALIFICATIONS TCDSS Proposal Number: 2012-CBCAP RFP NOTICE IS HEREBY GIVEN that the County of Tehama, Depart- ment of Social Services,(TCDSS) announces its intention to con- tract for certain services which will be provided through the Community Based Child Abuse Prevention (CBCAP) program. These funds will be used to pro- vide services to Tehama County families who have recently suc- cessfully reunified and closed their Child Welfare Services case. TCDSS will develop contracts with individuals and/or agen- cies, either working alone or in collaboration with other agen- cies, to provide "Functional Family Therapy" services, as de- scribed at the FFT website: http://www.fftinc.com/about_m odel.html. All services should be available to the diverse popula- tions of Tehama County. Private and public agencies or individuals interested in provid- ing the service listed above must submit a letter stating such interest to ATTN: Sue Schild, Tehama County Depart- ment of Social Services, PO Box 1515, Red Bluff, CA 96080, or by hand delivery at 310 S. Main Street, Red Bluff, CA. The letter must contain a brief description of services to be provided and the party's experience in and/or qualifications for providing the service. At that time, all bids will be publicly opened, exam- ined and declared for construc- tion of: CITY OF TEHAMA 4TH STREET WATER MAIN REPLACEMENT PROJECT GENERAL WORK DESCRIPTION: The work to be done under this Contract consists of installing one water valve and 380 lineal feet ± of 6 inch diameter PVC water main, installing 4 new wa- ter services, directional boring of one water service under 4th Street (additive bid item), pave- ment replacement and aban- donment of the existing water main. The Owner will issue a Notice to Proceed on a date mutually agreed upon by the Owner and the Contractor. Work shall be completed within 10 working days after issuance of the Au- thorization to Proceed by the Owner. The contractor is to carefully ex- amine the site of the proposed work, and is to make his or her own determination of the scope of the work to be performed, in- cluding but not limited to the soil and existing pavement con- ditions and/or groundwater conditions to be encountered in performing the work, and he or she is to carefully examine these Contract Documents. BID SUBMITTAL REQUIREMENTS: No proposal will be accepted unless it is made on a Proposal form furnished by the City of Tehama. PREVAILING WAGES: Pursuant to Section 1770, and following, of the California Labor Code, the successful bidder shall pay not less than the prevailing rate of per diem wages as determined by the Director of the California Department of Industrial Rela- tions. The successful bidder shall post a copy of the applicable wage rates on the job site dur- ing the construction period. APPRENTICESHIP STANDARDS: In accordance with the provi- sions of Part 7, Chapter 1, Article 2, Section 1777.5 of the Labor Code of the State of California, the prime contractor shall be re- sponsible for fully complying with the provisions of this Sec- tion, as well as any regulations adopted by the Director of In- dustrial Relations, for all apprenticeable crafts or trades, and shall also assure compli- ance by his or her sub- contractors with respect to such apprenticeable crafts or trades.
