I'm almost 20 now. I can pet and play with small, quiet dogs, but larger ones strike panic in me. I cringe when I hear barking. I still have nightmares and sometimes feel phantom pains where I was attacked. Spending time with my par- ents' medium-sized dog doesn't help, especially since it has bitten people before, and my family still treats it like the best dog in the world. I never got professional help, because I was told shortly after the attack that I needed to ''get over it.'' I feel no one understands my fear because it's an American tradition to love dogs. Besides, I don't have money for therapy. What can I do? — Terrified Dear Terrified: People who tell you to ''get over it'' do not under- stand the extent of the problem. Your fear is not irrational. You were viciously bitten, and such a trauma is difficult to overcome. The fact that you can pet and play with small dogs is quite an achievement, all considered. If you regularly encounter larger dogs and wish to work on your fear, please know that low-cost therapy is available. I use it mostly in the summer, but it's checked on at least once a month. My neighbor there is married to a woman who is no longer friends with me, but we are civil to each other. I took great pride in my garden of perennials and enjoyed the fruits of my hard work. Last fall, I noticed the daylilies were pulled right out of the ground. She also dug up a hosta that had been in that spot for years — and looked great. She obviously does her dirty work when her hus- band isn't home. She simply helps herself to my garden. I went to the police, and they suggested I discuss this with the neighbor. The officer said if the police dropped by, it would be con- sidered a threat, and besides, the husband may not know anything. This has been going on for years. Every season gets more frustrating. I bought an outside camera (not installed) and left her a note saying I had contacted the police, but it hasn't made any difference. She ignores all the ''Keep Out'' and dous fan interest and is exactly the type of con- tent we strive to provide our viewers. (Satur- day, May 5); and Pueblo, Colo. (Saturday, May 19). I was so surprised — what does this mean, exactly? DEAR READ- ER: I agree that it's a strange- sounding con- cept, but sun allergy is a real phenom- enon. A sun allergy is an immune system reaction to sunlight. Sun allergy usually results in an itchy red rash after sun exposure. This is most common on the "V" of the neck, the back of the hands, and the outside surface of the arms and the lower legs. Sun allergies are trig- gered by the effect of the sun on natural chemicals in the skin. The sunlight changes the shape of the chemicals, which makes them look "foreign" to the immune system. As a result, the body activates its immune defenses. The resulting allergic reaction can take the form of a rash, tiny blis- ters or, rarely, some other type of skin erup- tion. Sun allergies occur only in certain sensitive people. They can be trig- gered by only a few brief moments of sun expo- sure. Scientists don't know why some people develop a sun allergy and others don't. There is evidence, however, that some forms of sun allergy are inherited. For instance, people of American Indian background, including the native pop- ulations of North, South and Central America, have a higher rate of inherited sun allergy. There are a variety of types of sun allergy. The most common is poly- morphous light eruption (PMLE), which usually appears as an itchy rash on sun-exposed skin. But some people have more severe sun-related symptoms. This can mean hives, blisters or small areas of bleeding under the skin. In cases Blinds Need Blinds? Some- times additional tests may be nec- essary. People with a sun allergy must protect their skin from exposure to sunlight. Wearing sunscreen is a must for all exposed areas, including the lips. It should have an SPF of 15 or above and protect against both ultraviolet A and ultraviolet B rays. Those with a sun allergy should limit time spent outdoors when the sun is at its peak and also wear long pants, a shirt with long sleeves and a hat with a wide brim. Red-flag symptoms, such as developing hives along with swelling around the eyes or lips, faintness, or difficulty breathing or swallowing, can be signs of a life- threatening allergic reac- tion that requires imme- diate medical attention. Finally, some rashes that occur with exposure to the sun are caused by medicines. For example, tetracyclines, sulfon- amides (sulfa drug antibiotics), thiazide diuretics and fluoro- quinolone antibiotics can cause rashes when skin is exposed to sun. The way to deal with all of these problems is the same as how to pre- vent aging of the skin and skin cancer: Protect yourself with sunblock and clothes. This just becomes more important if you have a sun allergy, or are taking a medicine that makes you sun-sen- sitive. Dr. Komaroff is a physician and professor at Harvard Medical School.
