Making excuses for obesity DEAR DR. GOTT: My 38-year-old granddaughter is con- vinced that because she has polycystic ovarian syndrome (PCOS), she cannot lose weight. She is grossly overweight, and I am concerned about her future. She sees an endocrinolo- gist about once a year, and takes thyroid medication and niacin for cholesterol. Can she be helped? Any carb intake might focus on choices high in fiber, such as brown rice, whole-grain breads, fortified cereals and beans. Dr. Peter Gott DEAR READER: PCOS is a hormonal disorder that com- monly begins when a female begins menstruation; however, it can develop later in life. Obesity presents in nearly half of all patients with PCOS and is often the initial com- plaint. Abnormalities in the menstrual cycle, infertility, pre or type II diabetes and the presence of polycystic ovaries diagnosed through ultrasound are but a few of the other symptoms. It should be noted, however, that having polycys- tic ovaries is not a firm indica- tion of PCOS. There is no specific testing for diagnosis. Rather, it is one of exclusion, meaning that a physician will rule out a num- ber of related possibilities prior to deciding on PCOS. The disorder makes other con- ditions, such as type II dia- betes, hypertension and sleep apnea, more likely to occur. Only you and your grand- daughter can determine the sequence of events. When was she diagnosed? Could the PCOS be a result of her obesi- ty? Is that issue under control? I cannot tie in high-cholesterol levels with her excess weight; however, thyroid disorders are often linked with obesity, ele- vated cholesterol and menstru- ation abnormalities. There is help and hope that focuses primarily on her major symptoms. She should exer- cise and become involved with a regular weight-control pro- gram. Diet modification appears essential. There is some initial evidence of success on a low- carbohydrate diet without dis- crimination between carbs on either end of the glycemic index. Her caloric intake should be reduced. She should avoid simple carbohydrates such as soda, sugary drinks, Her physician might decide to place her on low-dose birth control as a means of regulating her menstrual cycles. She should continue to see her primary-care physician and endocrinologist on a regu- lar basis. DESCRIPTION OF WORK 99W OVERLAY PROJECT An overlay of portions of 99W IN Tehama County. There are two project segments, from Corning City Limits to Finnel Avenue and from Sunbright Avenue to Red Bluff City Limits. Project in- cludes replace asphalt concrete surface, a two-lift HMA overlay with pavement fabric, striping, and shoulder backing along the road. This project is bid as a Base Project with several additive al- ternatives which may be added depending on the cost of bids for the Base Project and each al- ternative. Approximately $1.1 Million is available for this proj- ect funded from the American Reinvestment and Restoration Act (ARRA). Additionally, the County may use its discretion and supplement the budgeted amount with other available County funds. The specific Funding Amount upon which the contract will be awarded will be publicly disclosed by the County before the first bid is opened, and will be announced at the bid opening. The County will evalu- ate all bids for this project based on the Base Bid, followed by the Base Bid plus an Alterna- tive or Base Bid plus combina- tion of Alternatives depending on the cost in relationship to the specified amount.
